ALUMNI HALL OF FAME NOMINATION INFORMATION

The Carrollton Schools Alumni Foundation is seeking nominations for the 2010 Alumni Hall of Fame. The nomination forms are due by April 1, 2010. The Outstanding Alumni Awards will be given to honor former students whose professional achievements and/or significant contributions to society, strength of character and citizenship serve as models to inspire and challenge today’s youth. The Outstanding Alumni Award shall be open to any alumnus or alumna from the Carrollton Exempted Village School District who graduated at least 10 years prior to the nomination. Awards may be given posthumously.

Awards for alumni will be given for achievement in various fields of service which include, but are not limited to, the arts, athletics, business, community service, education, entertainment, health, industry, law, medicine, military, music, philanthropy, politics, public service, religion, scholarship, and science. The Awards will be presented at the annual banquet, which will be held October 9, 2010 at the Day’s Inn in Carrollton.

Nomination forms may be picked up at the Carrollton Exempted Village Schools’ Board Office, or the Carroll County Library in Carrollton or Malvern. The nomination form may also be downloaded from the Carrollton Schools website at www.carrolltonschools.org. Click on the alumni link, located on the left side under main links, which is this web page then click the nomination form link below. You may also call the CEVSD Board office at 330-627-2181 to request a nomination form to be mailed to you. Any person may submit nominations.
Outstanding Alumni Awards Program Nomination Form

The Carrollton Schools Alumni Foundation Inc. is a group which allows any person to help out Carrollton High School via tax-deductible contributions. This foundation is operated exclusively for educational purposes. All funds collected will be used for curriculum enrichment, special projects, programs, and related uses for the benefit of public school children in the Carrollton Exempted Village School District.

You Can Help the Alumni Foundation continue to expand its support of our communities' public schools. The foundation's financial goal is to create a large endowment that will fund, in addition to annual grants, major and long-term programs. Your tax-deductible contribution can be made in any amount at any time during the year.

Please help Carrollton High School continue to be a leader in education and contribute.

Membership Rights and Application

Lifetime Membership ($500) -- One-time membership purchase for life. Receive a laminated certificate. Alumni vote on election of trustees at annual meeting.
Sustaining Member ($25 or more) -- Alumni vote on election of trustees at current year's annual meeting.
Contributing Member (under $25) -- Alumni not eligible to vote but they may attend annual meeting.
Associate Member -- Non-alumni wishing to contribute - Individuals, Institutions, Organizations or Corporations.
